I’ve had some rough semesters, and yes, I’ve had 50% or higher DFW rates. In those cases, I have not had the school coming to me with questions, because they knew about the issue long before final grades were assigned. I regularly touch base with my department chair (and upstream administrators, as appropriate). I make adjustments to help students who want help. I keep extensive records. I reach out to students repeatedly.
